What PSC officers look for
- Self-closing fire doors that don't close fully or latch
- Doors held open with wedges, hooks or ropes
- Damaged seals, door closers or hold-back magnets
- Unauthorised openings or penetrations in A- or B-class divisions
- Release from the bridge or fire control station not working
Typical root causes
- Closers worn or never adjusted; not in the PMS
- Doors propped open for ventilation or convenience
- Cable or pipe work by contractors without restoring the insulation
- Rounds not checking that doors operate
Corrective action (example)
Repair or adjust each closer and latch; remove hold-open devices; restore the insulation of penetrations with approved materials; test all fire doors and record the results.
Preventive action (example)
Add weekly fire-door operational checks to the PMS; brief crew on why doors must stay closed; add 'fire doors' to the hot-work and contractor close-out checklist.
Evidence to attach for close-out
- Before and after photos for each door
- PMS job record with test results
- Material certificates for penetration seals
- Crew briefing record
