What PSC officers look for
- Detectors faulty, disabled or isolated without a record
- Panel shows faults or disconnected loops
- Alarms not audible in all required spaces
- No test records, or tests not covering all detectors
Typical root causes
- Detectors isolated during hot work and not reinstated
- Ageing heads and no spare parts
- Testing done by sampling only, without rotating coverage
Corrective action (example)
Repair or replace faulty detectors; clear panel faults; reinstate isolated zones; test the whole system and record the results.
Preventive action (example)
Add an isolation log with sign-off for reinstatement; plan rotating detector tests so each head is covered within the PMS cycle; check spares stock.
Evidence to attach for close-out
