What PSC officers look for
- Fire pumps that fail to start or maintain pressure on test
- Leaking glands, corroded piping or seized valves on the fire main
- Emergency fire pump not ready for immediate use
- Isolation valves locked or marked incorrectly
- Fire hoses and nozzles not stowed for rapid deployment
Typical root causes
- PMS intervals for pump tests not followed
- Corrosion in seawater systems not trending in CMMS
- Spare parts for pump seals not on board

Corrective action (example)
Repair leaks and defective valves; prove both fire pumps on load test; label and verify isolation valve positions; restow hoses and international shore connection.
Preventive action (example)
Align PMS tests with company frequency; trend pump performance; include fire-main walk-down in port-state prep checklist.
Evidence to attach for close-out

- Photos of repaired piping
- PMS work orders
