What PSC officers look for

- Temporary covers left on intakes or exhausts
- Combustible deposits inside ducts
- Inadequate separation between exhaust and intake openings
- Missing flame screens on fuel tank vents where required
Typical root causes
- Operational habit of leaving fans running for comfort
- Cleaning of ducts not in PMS

- Poor handover between crews on fan stop procedures
Corrective action (example)
